What happens when you submit

We collect the answers and contact information shown in the form. If a suitable approved relationship is active, we may share the request with one licensed insurance professional or approved coverage partner that can follow up about health coverage.

The final form shows the current consent language before submission. Consent is not a condition of buying insurance, and you can use HealthCare.gov directly without submitting our form.

How we may be paid

HealthCoveragePath may receive compensation when an approved partner receives a lead or when another agreed event occurs. Compensation does not increase the premium established by an insurer, but the professional who contacts you may not represent every plan available in your area.

No enrollment or eligibility guarantee

Submitting a request does not apply for, bind or enroll you in coverage. A partner cannot guarantee eligibility, a tax credit, a particular premium or savings. Confirm plan details and official Marketplace eligibility before enrolling.
